One question remains unanswered: what do they write down their scrachpads? The true therapist possibly notes «Totally nuts!!!» (as a colleague does in a Gerry Larson cartoon) and the patient perhaps «Totally nuts!!!».

The first rule you have to learn as a cartoonist is: you shall not explain your cartoon! Never! NEVER! Why? When you do that, everybody believes it NEEDS an explanation and the punchline wouldn't work without. And in that case it would be a wordplay and not a cartoon.
... but: in this case here it would be funny to explain that the punchline REALLY stucks! Hahaha!
